Prepare For Your Visit
We have provided patient forms to assist you in preparing for your visit at Zehr Center Orthopaedics.
In order for us to prepare appropriately for your visit, it is your responsibility to:
- Obtain any necessary referral documents from the referring physician. For many commercial insurances and Medicare a referral from your personal physician is not necessary to be seen by our physician.
However, if your insurance requires a referral, please bring this document with you or arrange to have your physician's office fax it to Zehr Center for Orthopaedics at 239.596.6737.
- Obtain pertinent medical records, x-rays, MRI, or scans previously completed for this condition.

- Complete the Patient Medical History Form and bring it to your appointment (to print copy click on PDF below). Usually our nurse will contact you prior
to your first visit with us.
The form will help us to get this medical information into our electronic medical record prior to seeing you. This makes your visit more efficient.
- Review and sign the Financial Policy and bring it to your appointment (to print copy click on PDF below).
- Review the Notice of Privacy Practices (to review copy click on PDF below).
- Bring your insurance card(s) to your appointment.
- Bring your insurance copayment which is due at the time of service. For your convenience we accept cash, checks, money orders, debit cards, VISA, MasterCard, American Express and Discover.
